Program Overview
Primary care is the foundation of longitudinal health management. We provide acute and chronic care for a broad range of medical conditions, including hypertension, diabetes, hyperlipidemia, thyroid disorders, respiratory infections, urinary tract infections, and musculoskeletal complaints.
Our practice model uses telehealth to increase access, reduce barriers to care, and allow more time for each clinical encounter. This format is well-suited for medication management, lab review, follow-up visits, and ongoing coordination with specialists and pharmacies.
Evaluation and treatment of common acute conditions including upper respiratory infections, urinary tract infections, sinusitis, and skin infections.
Ongoing management of hypertension, diabetes, COPD, asthma, and other chronic conditions with lab monitoring, medication adjustment, and care coordination.
Age- and risk-appropriate screening for cardiovascular disease, diabetes, cancer, and sexually transmitted infections per USPSTF guidelines.
Comprehensive medication review, prescription management, drug interaction screening, and coordination with pharmacies including compounding pharmacies.
